Help with part number and availability please.

i appear to have a problem with my number plate light system.
I had a bulb fail, so removed the appropriate cover to discover that the contacts in side had melted into the plastic holders and made it impossible to make contact with the bulb holder contacts.
Only one side had done this, the other bulb works fine.
Obviously this needs to be replaced, what is the part called and is it easy enough to do it myself.
I am guessing I need to remove it by going into the boot lid via the inside of the car, as I can't see any obvious ways outside.
