BKD 2.0tdi Cam Belt

Hiya, just after some advise guy.
The cambelt on my car came off last night, didn't snap. Looks like the tensioner hasn't been fitted properly or has collapsed because its out of alignment with the other pullys and you can see its not straight.
2 things, the belt was changed April 2013 at 37k its now got 50k on, should I go back to who fitted it and if they'll take any liability, either for workmanship or part failure.
Also, do BKD's make a mess when the belt lets go???? It's not something I've come across on these engines, I'm going to try timing it up and fitting a belt to it to see if it turns without concern but wanted to know if anyone has any info weather I'm looking at a cylinder head??

The studs threads seem to let go often, I know of an engine builder who lives at the local Audi and VW dealers heli coiling the stud threads on engine blocks, too fine a thread in an aly block
