    When you say EPC, do you mean a little coil light? If so you have a diesel, if you do short journeys all the time and not many long journeys the DPF will get clogged up. This causes the car to go in limp mode. Give it a good long drive down the motorway every week or so and it should be okay.

    It is not sealing correctly, it could be warped, I wouldn't be able to tell just from the video. But check the rubber on the cap, make sure it is not damaged.
