Private Plate Help Needed

Hi,

I have recently just got my V5 back for my S3 and I'm wanting to put my private plate on.

When I got it bought for me, It was all taken care off to put it on my previous car so this is the first time for me having to do it. It's on retention now so is it just a case of me filling in that form or do I need a V62 aswell..?

Your help will be much appreciated....

DVLA usually put all the info required in together, including help notes.
