    Take a fuel sample from the feed pipe to the fuel filter. I take it you have also checked for fault codes?.

    Its possible the breather system has a leak. Check the pipes at the rear of the cam cover for hissing. sometimes they go really soft and can collapse.

    sounds like a fuel issue. Common problems are tandem pump leaking internally causing oil contamination in fuel (take sample). fuel leaking passed injector seals and injector seats ovaled on cylinder head

    check the sensor and see if its black or green. Change it if it is black. The temp gauge does not give a true temp reading.     coolant temp sensor is likely cause of this. If its reading incorrectly and thinks the engine is warm when it is actually cold this will stop the fast idle causing the judder.

    If you have the wind deflector trims fitted to the rear suspension then check that these are secure. The noise that they make you would swear something serious was wrong

    yes there is a known issue with the propshaft. The vibration is caused by the damper were it mounts to the haldex seizing

    Only when starting?. Had this issue a few times and there is a known fix for it. Its due to the current draw during starting affecting the level sensor and involves fitting a relay on to the sensor signal wire to dash panel which disconnects it under cranking.
