Private reg transfer paperwork issue

Hi all,

Can anyone that has transferred a Personal reg from car to car help me...
Im currently trying to transfer my private reg from my Mercedes vito to the A3 but when filling out the form, on the left hand side it asks you to fill the details in for the vehicle the registration plate is coming off,but I am un-sure if you put the original number plate details in or the private reg? can anyone shed some light on this?

The car its coming off (ie the reg you want) and the car its going onto.

Stick both tax discs where shown along with any MOT and the car having the plate taken off, will get a new reg or its original back on.
